Once a patient is registered as a User on the Patient Portal, they can access the Patient Portal at any time to access their Clinical Summaries and educational resources.
Go to www.MySecureHealthData.com.
The patient enters their e-mail address in the USER NAME field and clicks the SUBMIT button.
The patient enters their PASSWORD and ANSWER to their Secret Question, then clicks the SUBMIT button.
Once the patient is ready to exit the Portal, they can click the LOG OUT command.
Family members may share an E-Mail account and still access the Patient Portal. (V11.0.9)
When accessing the Portal, enter the shared E-Mail address and click Log In.
Then enter the birthdate and First Name of the person that you are accessing the Portal for and click Next.
Finally, enter the Password and the answer to the Secret Question and click Log In.
If a patient can't remember their Password or Secret Question, the Patient can request a Password Reset or your office can reset their Portal account.
Please note that an 'Office Reset' clears any previous CCDs or Educational Resources from their portal account.

There is a CONTACT US command on the Portal menu that sends an e-mail to the Portal Administrator who will assist the patient.
